Pre-anesthetic blood tests are necessary to increase the safety of your pet during anesthesia. These blood tests can help us detect anemia, dehydration, diabetes, kidney, liver and heart disease. All of these conditions can contribute to complications in anesthesia and surgery **Pre-anesthetic blood work will be mandatory if your pet is seven years or older. **

EKG is a noninvasive procedure. Electrodes are placed on your pets skin and connected in a specific order to a machine that, when turned on, measures electrical activity all over the heart. Results are sent to a specialist to be reviewed to make sure the patient’s heart can withstand anesthesia and surgery.

Recommended by our veterinarians, Dental Radiographs are images of the interior of the teeth and gums used to identify problems or other abnormalities.

Dental radiographs are vital in veterinary medicine because they:

1. Detect hidden dental issues like abscesses and fractures.
2. Assess tooth and bone health, revealing bone loss and periodontal disease.
3. Guide treatment plans for saving or extracting teeth.
4. Monitor treatment progress, ensuring effectiveness.
5. Detect oral tumors and other pathologies.
6. Facilitate early detection of problems for preventative care.
7. Ensure comprehensive dental care by thoroughly evaluating oral health.

*Please note that dental radiographs are mandatory in the event dental extractions are needed.

The Veterinarian may determine that dental extractions are necessary. If so, how would you like us to proceed?

We are unable to reach you at the time of the procedure, your pet will be removed from anesthesia and extractions will not be performed. If a second anesthetic procedure becomes necessary to complete dental care, all associated costs will be the responsibility of the client.

• I authorize and direct the veterinarians of Animal Care Center and Pet Care Center to perform the procedures and additional diagnostic and/or other treatment as deemed medically necessary for my pet. The nature and purpose of the surgical procedure(s), including the benefits, potential risks, and alternatives has/have been explained to me and no guarantee has been made as to the results and/or cure.
• I authorize the use of anesthesia and administration of pain-relief medications as deemed necessary by the veterinarian.
• I understand that there are inherent risks involved in any surgical procedure and that unforeseen complications may arise.
• I understand to prevent additional anesthetic procedures and gum disease, if noted, deciduous teeth or “baby teeth” may be extracted at an additional cost per tooth.
• I understand that I am responsible for all costs associated with the surgical procedure and any subsequent treatments, including those deemed necessary for medical or surgical complications and any other unforeseen circumstances that may arise.
• I understand that any estimates or charges for the planned procedure(s) are only approximations, and the final bill may be greater or less than these amounts.
• I understand that all services must be paid for in full at time of discharge.
• I understand that a deposit may be required before any surgical procedure(s) is performed.
• I understand that an I.V. catheter will be placed to have venous access during the procedure which will require the area to be shaved.
• I, the undersigned, certify that I am the owner, or authorized agent, of the animal listed above and I hereby consent to the use of anesthetic agents, pain-relief medications and surgical procedures described and agree with all forgoing statements.
